In all cases, scrub typhus was definitely diagnosed by Weil-Felix OX-K agglutination test and immunofluorescent antibody test.
Many of the recent reports of scrub typhus and other rickettsial diseases from the Indian sub-continent are based on clinical findings and the relatively nonspecific Weil-Felix test (5) including the study by Kamarasu and co-workers (9).
Sixteen of 31 serum samples from patients with suspected scrub typhus had titers 40-160 on Weil-Felix agglutination test with Proteus OXK antigen in 2003.
Patients negative for malaria, were tested by Weil-Felix test.
Weil-Felix tests on day 12 showed the serum to be positive for Proteus vulgaris OX19 (titer 160); tests for P vulgaris OX2 and OXK were negative (titer of 10 for both).
These diagnoses were made according to the clinical features of the illness and the reactivity of the serum samples to OX19 in Weil-Felix tests.
