Invasive fungal sinusitis can be classified as acute, chronic and chronic granulomatous, and tends to be encountered more in the immune-compromised population, particularly the acute form, which is rapidly progressive and may demonstrate bone erosion, vascular invasion and intracranial extension.
In 1996, Gucalp et al reported a case of invasive fungal sinusitis in an immunocompromised patient with acute myeloid leukemia; the pathogen, which was identified as P lilacinus on culture, was resistant to amphotericin B, but the patient was effectively treated with itraconazole and surgery.
This results in therapeutic confusion, since invasive fungal sinusitis is a completely different clinicopathologic entity from AFS and requires far more aggressive therapy, including toxic antifungal agents such as Amphotericin B.
Whereas invasive fungal sinusitis is more common in immunocompromised patients, mycetomas are more often seen in immunocompetent patients.
