The last one that he recalled occurred in May 1997, when a 400-gallon tank containing the plutonium stripping agent hydroxylamine nitrate exploded and exposed workers to a toxic plume.
Investigators concluded the blast occurred after two chemicals - nitric acid and hydroxylamine nitrate - were left in a tank for four years and became volatile as a result of evaporation, leaving an unstable mixture.
The tank contained a plutonium stripping agent known as hydroxylamine nitrate and gaseous nitric acid, which are toxic but not radioactive.
A preliminary investigation found that the explosion in the 408-gallon tank was caused by evaporation of a liquid solution of hydroxylamine nitrate and nitric acid, creating nitrogen gas.
A preliminary investigation concluded the 408-gallon tank contained less than 40 gallons of a liquid solution of hydroxylamine nitrate and nitric acid, said Ron Gerton, chairman of an Energy Department team investigating last Wednesday's blast.
The explosion occurred in a metal tank containing hydroxylamine nitrate, an oxidant similar to salt.
