Dynamic hyperinflation contributes to increased elastic and mechanical loads on the inspiratory muscles and to neuroventilatory dissociation which further exacerbate the shortness of breath, leading to exercise intolerance, limited physical activity, and thus to a poor quality of life (Christopher 2006, O'Donnell 1994, O'Donnell et al 2007).
Background The authors indicate that patients with COPD are limited during exercise by dyspnea, which is related to both an increase in respiratory muscle work and dynamic hyperinflation.
The authors suggest smaller airways, dynamic hyperinflation, inspiratory muscle weakness, or increased stiffness in the lungs as potentials causes.
Gas trapping and dynamic hyperinflation leads to a higher end-expiratory pressure known as auto-PEEP or intrinsic PEEP.
3) Mechanical ventilation in patients with obstructive lung disease carries the hazard of dynamic hyperinflation, with its sequelae of hypotension and barotrauma.
Dynamic hyperinflation has been identified as a possible mechanism which limits exercise in patients with COPD (O'Donnell and Webb, 1993).
The signs suggesting dynamic hyperinflation include: 1) the patient is still exhaling at the moment the ventilator delivers the next breath, 2) hyperinflated chest exam, 3) hemodynamic effects such as tachycardia and hypotension, and 4) the patient's respiratory efforts are unable to trigger a ventilator-assisted breath (auto-PEEP).
[FIGURE 3 OMITTED] Indices of dynamic hyperinflation during exercise It has been established for some time that DH occurs in flow-limited patients under conditions of increased ventilatory demand during exercise (Grimby et al.
